Facility Evaluation Report
Licensing Program Analyst (LPAs) Mia Lankford and Ivashia Wright is conducting this unannounced case management visit on this date to check on the health, safety, and welfare of clients in care. LPAs learned that four (4) clients currently reside at this facility. Nine (9) staff is currently present. LPAs met with O'Koia Nixon- Assistant Administrator and annouced the reason for the visit.
During this visit LPAs toured the facility on the inside and outside. LPAs assessed the food supply and found that it exceeds the minimum requirement of a 2-day supply perishable and a 7-day supply of non-perishable foods. Utilities are on and functioning, LPAs observed sufficient staff for care and supervision of clients.
Based on the information obtained today LPAs does not observe any immediate threat to the health, safety, and welfare of the clients residing at this facility. There are no deficiencies being issued per Title 22, Division 6 of the California Code of Regulations.
This report was reviewed with O'Koia Nixon and copy provided to the facility representative at the time of exit interview.
