Facility Evaluation Report
On 06/17/2026, Licensing Program Analyst (LPA) Mia Lankford conducted an unannounced visit to the facility to conduct a required annual inspection. LPA met with Licensee Shawntae Nelson who was informed of the purpose of the visit and was granted entry.
LPA toured the facility with the Licensee Shawntae Nelson and reviewed 2 clients records and 4 staff files. There are 2 clients in care. LPA observed the facility is made up of a one-story home with four (4) client bedrooms and two (2) bathrooms, a living room, dining room, kitchen, and attached garage. All client bedrooms had the required furniture, lighting, and closet storage. LPA toured the facility's exterior and observed outdoor pathways were free of obstructions. LPA toured the kitchen and observed the facility has a seven-day supply of non-perishable foods that were observed to be properly stored and readily available. Knives and sharp instruments were secured in a locked kitchen cabinet. Fire Extingusher was service on 8/7/2026, water temp is at 16 degrees, home temperature is at 72 degrees and Administrator certificate expires 1/20/2028.
LPA observed Medications and disinfectants located in the hall closet locked. LPA observed activity center in the living room.
During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted and a copy of this report was reviewed and provided to Licensee Shawntae Nelson..
