Facility Evaluation Report
On 04/25/2025, Licensing Program Analyst (LPA) Debbie Palacios conducted an unannounced one (1) year required visit. LPA was granted entry by Administrator Shawniqua Davis, who was informed of the purpose of visit. At the time of the visit, LPA was informed that all clients were attending the day program and were not present at the facility. LPA observed the following during today's visit:
LPA conducted a tour of the facility with Administrator Shawniqua Davis. The physical plant is a single story structure that contained four (4) client bedrooms, one (1) staff bedroom, and two (2) bathrooms. The facility has a dinning room, kitchen, living room, staff office, garage, and a gated backyard. Indoor and outdoor passageways were free of obstruction. There were no bodies of water located on the property. LPA toured the kitchen and observed that the facility has a two (2) day supply of perishable food and seven (7) day supply of non-perishable foods. Knives and sharp items were observed in a locked container in the pantry closet. Extra linen, towels and blankets were observed in the closet located in the hallway. Client's bedrooms had the required bedding, furniture, and lighting. LPA also observed the facility has a locked storage cabinet in the laundry room filled with additional cleaning solutions, disinfectants, and laundry detergents. Administrator tested the smoke and carbon monoxide detectors and were observed to be operable. Centrally stored medication were observed in a locked cabinet in the kitchen. The outdoor patio was observed to have shaded seating to encourage outdoors socialization. Two (2) fully charged fire extinguishers were observed in the facility last serviced on 11/20/2024. The living room was observed to have board games and other activities. The facility was observed to be in a clean condition; free of dirt, insects, rodents, and pests.
