The inspector’s account

Licensing Program Analyst (LPA) Mary Flores conducted an unannounced case management visit during a complaint investigation visit to note observed deficiencies. LPA met with Shannon Moore and explained the reason for the visit.

On 12/3/25 LPA Flores conducted a tour of the facility LPA noticed a strong urine odor while walking in the hallways coming from the restrooms located near room D12, H27, E17. Per administrator they are currently using environmental friendly cleaning supplies which does not help with the smell.

Deficiency was noted on LIC 809D per Title 22 Regulations.

Exit interview was conducted with Shannon Moore and a copy of this report, LIC 809D, and appeal rights were provided.
