Facility Evaluation Report
Licensing Program Analyst (LPA) Mary Flores conducted an unannounced case management visit regarding deficiencies found during a complaint investigation. LPA met with Brittney Walsh Business Director and explained the reason for the visit.
On 4/5/25 LPA Flores conducted a complaint investigation during the investigation it was found there were no incident reports for Resident's #2(R2) sustained between October and December of 2022.
On 10/13/22, R2 was taken to the hospital due to a hematoma.
On 11/7/22 R2 was seen at the hospital due to a mechanical fall.
On 12/13/22 R2 was seen by a physician due to a fall.
Deficiencies are noted on LIC 809D per Title 22 Regulations.
Exit interview was conducted and a copy of this report was provided.
