Licensing Program Analyst (LPA) Kimberly Ramirez conducted a Case Management Visit-Deficiencies on 03/29/2025, stemming from subsequent complaint investigation on 3/29/2025. LPA Ramirez was greeted by Staff Brittney Walsh and explained the purpose of the visit.

On 3/29/2025, LPA Ramirez attempted to obtain copies former resident (R1) and former staff (S1) records for an open complaint investigation. Facility staff revealed documents requested were not available to LPA Ramirez. Staff revealed documents requested are in storage and the facility will need more time to get the documents. Per Title 22, Division 6, Chapter 8, Article 9. Resident Records- 87506(d) Resident Records - All resident records shall be available to the licensing agency to inspect, audit, and copy upon demand during normal business hours. Records may be removed if necessary for copying. Removal of records shall be subject to the following requirements:. LPA Ramirez will issue a Type B violation based on this observation. Per Title 22, Division 6, Chapter 8, Article 07. Personnel Records-87412(g) Personnel Records- All personnel records shall be maintained at the facility and shall be available to the licensing agency for review. LPA Ramirez will issue a Type B violation based on this observation.

Two (2) violations were observed and cited. Exit interview was conducted. A copy of this report, 809-D and appeals rights was provided.
