Facility Evaluation Report
On 10/09/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required inspection. LPA was greeted and granted entry by DSP Jasmine Espinoza where LPA explained the purpose of the visit, the administrator Rasheedah Simmons arrived shortly after and accompanied LPA on a tour of the interior and exterior areas of the facility. During today's visit LPA obtained updated contact information for the facility. The facility is licensed to serve age range 18 through 59, approved for 4 ambulatory clients. The facility was observed to be clean and clutter free. The facility is equipped with solar panels and was observed to have personal protective equipment (PPE) supplies, emergency food, personal hygiene supplies and extra linens. The hot water temperature was tested and found to be within regulatory limits measuring at 114.3 degrees Fahrenheit. The combined spoke and carbon monoxide detectors were tested and found to be operable. The fire extinguisher is fully charged, and was last serviced on 10/01/25.The sharps, chemicals and other hazardous items are locked and inaccessible to clients in care. There are no guns, ammunition or bodies of water on the premises. The emergency disaster drills are being conducted on a monthly basis, with the last drill being conducted on 10/7/25. The food supply was sufficient as there is 2 day supply of perishable and a 7 day supply of nonperishable food items.
A records review was conducted of both staff and client files. All staff present were observed to have criminal record clearance and to be associated to the facility. Staff files reviewed were observed to have valid Crisis Prevention Intervention (CPI) as well as Cardio Pulmonary Resuscitation (CPR) training. The administrator was observed to possess a valid administrator's certification, which expires on 03/21/26. The governing body was observed to be in good standing. LPA gave a reminder of the annual fees are due on or before 10/21/25 and provided PIN 341759. Based on today's inspection no deficiencies were issued and the facility was evaluated in accordance with the California Code of Regulations (Title 22, Division 6, Chapter 6). An exit interview was conducted where a copy of this report, was reviewed and provided to Rasheedah Simmons, Licensee/administrator.
