Licensing Program Analyst (LPA) Seo Jeon made an unannounced visit to the facility to conduct a Case Management visit regarding a complaint report number 18-AS-20250303133359 . LPA spoke with the Executive Director, Valeria Garcia, and informed them of the purpose of the LPA's visit. LPA conducted a health, safety and welfare check of residents in care. LPA did not observe any health and safety concerns at the time of the visit.
There are no deficiencies being cited, per California Health & Safety Code and Code of Regulations, Title 22.
An exit interview was conducted, a copy of this report was provided to the Executive Director.
