Facility Evaluation Report
Licensing Program Analyst (LPA) Debbie Palacios made an unannounced visit to the facility to conduct a required annual inspection. At the time of arrival, there was no staff or clients at the home; LPA telephone contacted Director Lester Bell and staff member Maurice Bell arrived to the home at approx. 2:20 pm. LPA was greeted and granted entry by staff member Maurice Bell and Director Lester Bell arrived a few mintues shortly. Director Lester informed LPA that the facility does not have clients in care. The facility has a fire clearance for three (3) ambulatory clients and serves adults ages 18 through 59.
During the tour, LPA observed the facility is made up of a one (1) story home with three (3) client bedrooms, two (2) bathrooms, a living room, dining room, laundry room and attached garage. All client bedrooms had the required furniture and lighting. LPA toured the facility's exterior and observed outdoor pathways were free of obstructions. Outdoor shaded seating area is available for the clients in care. LPA observed that the laundry room has a cabinet filled with clean towels, blankets, and linen, available for the clients. LPA toured the kitchen and observed the facility has a 7-day supply of non-perishable foods, which are stored in a safe and healthful manner. LPA observed knives and sharp instruments secured in locked kitchen cabinet. Cleaning solutions and disinfectants are secured in a lock cabinet in the kitchen underneath the sink. Staff member Maurice tested one (1) of the smoke alarms/carbon monoxide detectors and LPA observed it to be operational. LPA also observed one (1) charged fire extinguisher mounted throughout in the kitchen wall dated 02/10/23. Medication will be stored in a locked kitchen cabinet. LPA observed required postings in the facility. No health or safety issues were observed during the time of the visit.
During today's visit, LPA did not observe any issues or concerns. An exit interview was conducted, and a copy of this report was reviewed and provided.
