Facility Evaluation Report
On September 15, 2025, Licensing Program Analyst (LPA), Seo Jeon made an announced visit for the purpose of the facility's closure and met with Catherine Omr, Administrator. LPA was granted entry into the facility for closure inspection.
LPA conducted a walk-through of interior and exterior areas of the facility. LPA did not observe any clients or any sign of clients residing in the facility. LPA confirmed with the Administrator that there has been no clients since January 15, 2025. LPA advised that the license was no longer valid. LPA also advised that a new application must be submitted to the Department in case this location is to be operated as a community care facility again.
LPA verified two (2) clients were relocated to another facility in December 2024. LIC812 was created with details for the client relocation. LPA did not observe any health and safety concerns during the visit. The Administrator will mail the original license to the Regional Office.
No deficiencies were cited during the visit. An exit interview was conducted, during which this report was reviewed and provided.
