Facility Evaluation Report
On 04/06/2026 Licensing Program Analysts (LPA) Ahliah Sharp conducted an unannounced annual required visit. Upon entry, LPA was greeted by Juan Simmons, Administrator (A1), and informed them of the purpose of the visit. A1 informed LPA that the Licensee would be on site shortly to assist with the annual. At the time of the visit, there was one (1) staff member, and two (2) residents present with one (1) at day program. Shortly after arriving, the Licensee arrived.
Facility Overview: The facility is a one-story home with (4) bedrooms and (2) bathrooms, including a detached garage. There are no pools or known firearms on the premises.
Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility’s infection control plan was reviewed and found to meet department’s requirements.
Physical Plant: The physical plant, including floors, windows, and doors, were clean and well maintained. Fixtures and furniture were all in good repair. The outdoor area was free of hazards and debris. Laundry was being done at the time of the visit, and the equipment was in good working condition. All sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector were operational, and the hot water temperature was 120.7°F, and hot water warning signs were posted in each area. Fire extinguisher located at dining area has current inspection tag dated 03/10/2026.
Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility exceeded the required minimum two-day supply of perishable foods and a seven-day supply of non-perishable foods.
