The inspector’s account

On 04/23/2026, Licensing Program Analyst (LPA) Tremayne Barra made an unannounced visit to the facility to conduct a required annual inspection. LPA was greeted and granted entry by staff member John Calimag. Lead Staff Adriano Aguinaldo joined us later and was informed of the purpose of the visit. At the time of the visit, there were two (2) staff members present at the facility and three (3) non-verbal clients. The facility has a fire clearance for six (6) ambulatory clients of which two (2) be non-ambulatory and serves adults ages 18 through 59.

LPA toured the facility inside and outside. LPA observed the facility to be clean and in good repair. LPA observed a one story home that consists of three (3) client bedrooms, one (1) staff bedroom, and two (2) bathrooms. LPA observed that the client bathroom has cabinets that stores clean towels, linens and blankets. LPA also observed a kitchen and dining area, a living room area, a garage, and a patio and yard with sufficient seating and space for activities. The home is maintained at a comfortable temperature for the clients. Lighting is sufficient for safety and comfort. Water temperature measured within regulation requirements. Garage has locked cabinets present for storing laundry soap and other chemicals. All outdoor and indoor passageways are free of obstruction. Emergency lighting is available. There is a telephone working at this location. The LIC 610, emergency disaster plan is maintained. No body of water was observed on the facility.
