The inspector’s account

On 04/17/25, Licensing Program Analyst (LPA) Debbie Palacios made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted by Caregiver Lourdes Pagulayan who was informed of the visit. The facility has a fire clearance for three (3) clients; two (2) ambulatory and one (1) non-ambulatory.

LPA toured the facility and reviewed records. During the tour, LPA observed the facility is made up of a two-story home with three (3) client bedrooms, one (1) staff bedroom, three (3) bathrooms, living room, kitchen, backyard and attached garage. All client bedrooms had the required furniture, lightning, and closet storage. LPA toured the facility's exterior and observed outdoor pathways were free of obstructions. LPA toured the kitchen and observed that the facility has a two-day supply of perishable foods and more than a seven-day supply for non-perishable foods that were observed to be properly stored and readily available. Knives and sharp instruments were secured in a locked kitchen cabinet. LPA observed that the centrally medications were stored in another locked kitchen cabinet. LPA toured the garage and observed a second refrigerator with additional perishable foods. LPA observed that the washer and dryer are located in the garage; staff keeps the garage door locked at all times. LPA reviewed two (2) client files and one (1) staff file. Client's files had the updated individual program plans and signed admission agreements. Staff file had the Department's required training records and valid first aid/CPR certification. LPA observed that the facility has two (2) Fire Extinguishers last serviced on 11/11/2024.

LPA reviewed the facility's Fire and Earthquake Drills and noted that the last fire and earthquake drills were conducted on 04/01/25.
