Facility Evaluation Report

On 08/25/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to conduct a 1 year required visit. LPA met with administrator Portia Garcia, where LPA explained the purpose of the visit. At the time of the visit there was one (1) staff present, and zero (0) clients. The facility does not have any clients at this time, or since they have been licensed since 08/21/23. Below are the observations made during today's visit:

The facility was observed to be be clean, and clutter free. The facility is equipped with emergency disaster supplies such as food, and water, personal protective equipment, personal hygiene supplies as well as linen. The rooms were observed to have the required furniture (bed, chair, lamp, chest of drawers). The facility has games, and coloring activities available for client use to encourage socialization.

Per a file review conducted the administrator was observed to possess a valid Administrator certificate which expires, on 06/22/26. Administrator also possess a valid CPR/First aid certification which expires on 11/08/26.

The fire extinguishers were fully charged, there are no drills being conducted as there are no clients in care. The smoke and carbon monoxide detectors were tested and found to be operable. The chemicals, and sharps were observed to be locked and inaccessible to clients in care. The facility annuals fees have not been paid that were due on or before 08/21/25. LPA provided PIN 319553, should the licensee wish to pay electronically. The governing body was observed to be in good standing.

Based on today's inspections no citations were issued, an exit interview was conducted where a copy of this report was reviewed and provided to Portia Garcia, Administrator.
