Complaint Investigation Report
An additional allegation alleges that staff do not keep the facility at a comfortable temperature. LPA observed facility temperature to be within regulation on multiple visits that occurred on 11/07/2025, 09/03/2025 and 02/13/2026. LPA conducted (2) client interviews. Interviews revealed the facility keeps temperature at a comfortable temperature.
An additional allegation alleges that staff do not meet residents’ hygiene needs while in care. LPA conducted (2) client interviews. Per interviews, there was not enough evidence to corroborate that allegation occurred. LPA conducted (2) staff interviews. 2 out of the 2 indicated hygiene needs are met at the facility and clients are assisted when needed.
The sixth allegation alleges that licensee is retaliating against residents in care. Per interviews, it was discovered Client #1 (C1) was given 30-day eviction due to increased conflict and unable fit for facility. LPA observed C1 was given time to move out of facility and was moved out sooner than 30-day notice. Based on observations and interviews, there is not enough evidence to corroborate that allegation of retaliation towards client occurred.
The seventh allegation alleges resident sustained unexplained injuries while in care. LPA interviewed Administrator who indicated C1 did not sustain any unexplained injuries while in care at the facility. Administrator stated facility staff would de-escalate and redirect as needed. Additionally, C1 stated facility staff has never caused injury to C1. Based on interviews, there is not enough evidence to corroborate that client received unexplained injuries while in care.
The eight allegation alleged that clients were not adequately fed while in care. LPA conducted (2) client interviews. 2 out of the 2 clients stated facility did ensure that clients were adequately fed while in care. Additionally, LPA observed facility equipped with more than seven-day supply of non-perishable food and more than 2-day supply of perishable foods on 11/07/2025, 09/03/2025, and 02/13/2026 located in kitchen refrigerator and pantry.
The ninth allegation alleges that staff did not prevent an altercation between clients in care. LPA spoke with Administrator who stated in the event facility staff would de-escalate situations and redirect clients in care. LPA conducted (2) staff interviews. 2 out of 2 clients stated in event altercation happened staff would intervene. Per interviews, it appears facility staff did appropriately address the altercation between both clients in care.
Based on the evidence gathered during today’s investigation, the allegation listed above are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. An exit interview was conducted and this report (LIC9099) along with other forms were discussed and provided to Administrator Oluwatoyin Oladimeji.
