Facility Evaluation Report
On 10/10/2025 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit to investigate open complaint 56-AS-20251006101335. During LPAs visit, LPA observed Staff #1 (S1) does not have a criminal background clearance. Upon observation, LPA discovered S1 is excluded to be working at any facility. LPA informed Administrator that a deficiency for having an excluded person working at the facility will be issued as this poses a health, safety and personal rights risks to clients in care.
An Immediate Civil Penalty of $500 will be assessed for Staff #1.
During today’s visit, a deficiency and civil penalty was cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and this report (LIC809), (LIC809D), (LIC421BG) was discussed and provided to Administrator Oluwatoyin Oladimeji along with a copy of the appeal rights.
