Facility Evaluation Report
On 04/02/2025 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unnanounced visit pertaining to complaint 56-AS-20221031141433 . During visit, LPA Hernandez observed Resident #2 (R2) facility file to not be located at the facility. LPA advised Administrator all resident's files must be at facility and presented to licensing upon request.
Based on the observations made during today’s visit, a deficiency was cited per Title 22, Division 6, of the California Code of Regulations.
An exit interview was conducted, and this report (LIC809) (LIC809D) was discussed and provided to Assistant Administrator Sandy Zhao. Along with a copy of the appeal rights.
