Facility Evaluation Report

Licensing Program Analysts (LPA) Mary Rico attempted to conduct an annual inspection on 10/22/2025. LPA met with staff member Giselle Donis and was granted entry to the facility.

Upon arrival, LPA Rico observed that the facility is no longer operating under Community Care Licensing. The facility is now operating under the California Department of Public Health for Congregate Living, under license number 550009788. LPA Rico also observed the official license issued by the State of California Department of Public Health.

LPA Rico spoke with the new owner, Mac Tang, and informed him that there had been no formal closure process completed by the previous owner. The prior facility license is still listed as active under Community Care Licensing.

The previous Administrator, Simm K. Sanashinh, was contacted and informed of the required procedures for proper closure. An exit interview was provided to the Administrator Simm K Sanashinh.
