The inspector’s account

Therefore we were unable to complete a full investigation.

Based upon this investigation, LPA finds that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is UNSUBSTANTIATED.

ALLEGATION #2 STAFF ARE SERVING FOOD THAT IS NOT OF QUALITY

It is alleged that food being served is not of good quality

On 10/1/2024 LPA Romero toured the facility's interior and exterior, conducted interviews and obtained copies of pertinent documentation. The facility was advised that the investigation requires further inquiry and possible additional visits and/or phone calls may be necessary to determine findings. An exit interview was conducted and a copy of the report was reviewed and provided to Administrator Scott.

On 3/13/26 Licensing Program Analyst Sparkle Day began the follow up investigation. LPA Day attempted to reach the reporting party and the facility at the numbers provided, however there was no contact made. The facility closed 5/8/25. Due to facility closing we were unable to locate all parties involved in the complaint

Therefore we were unable to complete a full investigation.

It is alleged that staff do not serve the food from the facility menu
