On 4-21-25, Licensing Program Analyst (LPA) Abdoulaye Zerbo conducted a Case

At 01:15 PM, when touring room 28, LPA observed feces smeared on the curtain, the AC unit and a foldable table. ED stated the resident often has the tendencies of smearing their feces on different objects. They also stated the resident will turn on the faucet and forget about it, which will flood the room and LPA observed damaged flooring by the closet due to the excess water. The ED stated they will have someone clean the feces and clean the room. This constitute a health and safety issue for the residents in care, because the room is shared by another resident.
An exit interview was conducted, and Appeal Rights were discussed with Executive Director Heather Scott . A copy of the LIC809, LIC 809D, and appeal rights was provided to the Administrator. Please see LIC 809-D for citations related to the deficiencies observed during the inspection.
