Facility Evaluation Report

Licensing Program Analyst (LPA),Abdoulaye Zerbo conducted an unannounced visit to the facility for a health an safety check. The LPA met with Executive Director (ED) Heather Scott , and informed them of the purpose of the visit and was granted access.

The facility is a single story building with Twenty eight (28) residents bedrooms, 28 bathrooms, an office, and a medication room. There is no gated pool and there are no firearms on the premises.

LPA's case management included interview with management, staff and resident, obtaining pertinent documentation and conducting a tour of the facility for a health and safety check. No health and safety concerns were observed during today's visit. According to records review and the ED, the resident has a history of elopement. Discussion were done with ED and they stated a tracking device will be purchased by the resident's spouse and a wall will be built to reduce the risk of elopement.

ED was advised that possible visits and phone interviews will be conducted before a decision is rendered. An exit interview was conducted, and a copy of this report was provided to Executive Director (ED) Heather Scott
