Facility Evaluation Report

Licensing Program Analyst (LPA) Janira Arreola conducted an unannounced visit in order to conduct a case management visit. LPA identified herself and the purpose of the visit. LPA met with Administrator, Kirk Hammond who was informed on the purpose of the visit.

The visit was conducted in response to a self reported incident submitted by the facility. LPA reviewed the submitted incident report, SOC341, and additional facility documents. LPA conducted client and staff interviews during the time of the visit. LPA conducted a health and safety check and a walk through of the facility during the time of the visit. There were no immediate health or safety issues observed.

Additional information may be requested on the incident. No deficiencies were cites at this time. An exit interview was conducted and copy of this report was reviewed and provided.
