Facility Evaluation Report

Licensing Program Analyst (LPA), Abdoulaye Zerbo conducted a visit to the facility for the purpose of a case management. LPA met with Administrator, Michele Hunsaker and informed them of the purpose of the visit.

On 05-27-25, Community Care Licensing received a serious incident report reporting multiple incidents involving Staff Member (S1) and residents. It was reported there was inappropriate interaction between staff and clients. During the visit, LPA conducted a tour of the facility. No health and safety concerns were observed. LPA also obtained pertinent documentation in regards to the incidents. LPA was informed by Administrator that S1 was immediately taken off the schedule once the incidents were advised of. It was further stated that due to additional information facility representatives obtained, S1 was terminated.

LPA advised Administrator that further investigation is still needed at this time.

An exit interview was conducted and a copy of this report was provided to Administrator Michele Hunsaker
