Facility Evaluation Report

Licensing Program Analysts (LPAs) Mary Rico and Antoinette Davis conducted an announced visit to the facility for the purpose of a change of ambulatory, non-ambulatory and bedridden. LPA was greeted and granted entrance by Administrator Edgar Gallardo..

Per the LIC200, Licensee requested for the to convert rooms 151-162 from independent living into assisted living units. Current assisted living rooms are 166-192,244-262. 264-292. The Licensee is also requesting to increase total bedridden occupancy. The facility went from (72) bedridden to (84) bedridden. The fire clearance request was approved on 5/9/2025.

The licensee was advised that the noted designated capacity and status for each room is to remain in compliance.

LPA observed that the residents’ bedrooms had functional lighting. The physical plant is ready. LPA will update the facility's file and issue a new license.

An exit interview was conducted where this report LIC809 was discussed and provided to Administrator Edgar Gallardo.
