Facility Evaluation Report

Licensing Program Analyst (LPA) Mary Rico conducted an unannounced visit to the facility to verify the clearance of the Plan of Correction (POC) issued during the visit on August 22, 2025. Upon arrival, LPA Rico met with staff members Mildred Manuel and Marionito Delacruz and was granted entry into the facility. Administrator Susan Nuguid was notified of the visit during its course.

The following Plan of Correction items were verified and cleared at the time of the visit:

· Section 87303(a): LPA Rico observed that the air conditioning unit had been repaired.

· Section 87303(a)(1): LPA Rico observed that the facility’s kitchen, bathroom, and hallways had been thoroughly cleaned.

· Section 87555(b)(27): The licensee provided documentation confirming pest control services.

· Section 87465(h)(5): LPA Rico verified that the Monday through Friday medication storage containers had been removed from the facility.

· Section 87465(c)(3): LPA Rico observed that the facility’s interior temperature was 30 degrees cooler than the outside temperature.

Facility will receive a Letter of Deficiency Citation Cleared for the deficiency cleared during inspection.
