Facility Evaluation Report

Licensing Program Analysts (LPAs) Mary Rico and Raquel Hernandez conducted an unannounced visit to the facility to verify clearance of Plan of Correction from visit on 10/9/2025. LPA Rico and Hernandez met with Licensee John Bulter.

The following Plan of Correction (POC)s were cleared at the time of the visit:

The Licensee was cited on 10/9/2025 for 85095.5(c) Infection Plan. During today's visit LPAs conducted a record review and observed an Infection Control Plan had been updated an completed. Therefore, the POC was cleared at today’s visit.

The Licensee was cited on 10/9/2025 for 80069(c) Medical Assessments. During today’s visit, the Licensee provided Medical Assessments for all clients. Therefore, the POC was cleared at today’s visit.

The Licensee was cited on 10/9/2025 for 85068.2(b)(1)(c) Needs and Services Plan. During today's visit, the Licensee provided a Care Plan for all clients.Therefore, the POC was cleared at today’s visit.

The Licensee was cited on 10/9/2025 for 1565(f)(2(d) Emergency Food. During today's visit, the Licensee provided proof of Emergency Food, water and supplies. Therefore, the POC was cleared at today’s visit.

The Licensee was cited on 10/9/2025 for 85064(b) Administrator Qualifications and Duties 85064(b). During today's visit, the Licensee provided proof of Administrator for the facility. Therefore, the POC was cleared at today's visit.
