Licensing Program Analyst (LPA) Mary Rico attempted to conduct an annual inspection. Upon arrival, Licensee Sheila Saginario indicated they had surrender their license with Community Care Licensing. The Licensee also stated clients were relocated by the assistance of Inland Regional Center. LPA conducted a closure visit.
LPA toured the facility and observed no client's belongings and no clients in care. The licensee stated they no longer interested in maintaining a license.
An exit interview was conducted where this report(LIC809) was discussed, and provided to Licensee Sheila Saginario.
