Facility Evaluation Report

Licensing Program Analyst (LPA) Mary Rico conduct an unannounced case management to obtain records from the facility pertaining to Compliant Control Number 56-AS-20251226110633 . LPA met with Administrator Crystal Cortez and was granted entry to the facility. LPA Rico met with Administrator Crystal Cortez and explained the purpose of the visit. The Licensee was also contact and informed about today's visit.

During today's visit, LPA Rico conducted staff and resident interviews. Based on the observations made during today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and this report (LIC809) was discussed and provided to Administrator Crystal Cortez.
