Facility Evaluation Report
Licensing Program Analyst (LPA) Mary Rico conducted an unannounced case management visit. LPA Rico met with staff Peter Santalla and explained the purpose of the visit. During an investigation pertaining to complaint control number 56-AS-20250716140942, it was relieved that S1 had handled C1 in a rough manner. During staff interviews 3 out of the 4 staff confirmed S1 handled C1 in rough manner. In addition, during staff interviews 3 out of the 4 staff confirmed that due to S1 handling C1 in rough manner, C1 resulted in scratches and bruises throughout their body. During client interviews, C1 confirmed that S1 had handled them in rough manner. LPA Rico obtained records that demonstrated that C1 had bruises throughout their body.
Based on the observations, record review, and interviews made during today’s visit, (1) deficiency were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and this report (LIC809)(LIC809D) and Appeal Rights was discussed and provided to staff Peter Santalla.
