Facility Evaluation Report
Licensing Program Analyst (LPA) Raquel Hernandez made an unannounced visit to conduct a Health and Safety check of the clients in care at the facility. LPA Hernandez met with Administrator Carlito Tan explained the reason for the visit.
The Health and Safety check included overall observation of the facility inside, and outside, including food supply, medications, physical plant, and the clients in care. When conducting medication audit, LPA observed for Client #1 (C1) medication was not properly administered. Deficiency will be issued. LPA observed only (1) client in care. LPA Hernandez did not observe any imminent health or safety hazards.
Based on the observations made during today’s visit, (1) deficiency was cited per Title 22, Division 6, of the California Code of Regulations.
An exit interview was conducted, and this report was discussed and provided to Administrator Carlito Tan.
