Facility Evaluation Report

On 03/06/2026 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit to facility in regards to complaint 56-AS-20251203115650. LPA met with Administrator Bene Molinitas and explained the purpose of today's visit.

The department received a complaint on 12/03/2025 in regards to Resident #1 (R1). Per investigation, it was founded that R1 was admitted to facility with a stage 4 pressure injury. The facility did not follow proper reporting requirements and did not report or submit a written exception request for prohibited health care condition to licensing department.

Based on the observations made during today’s visit, a deficiency was cited per Title 22, Division 6, of the California Code of Regulations.

An exit interview was conducted, and this report (LIC809) (LIC809D) was discussed and provided to Administrator Bene Molinitas. Along with copy of appeal rights.
