On 08/04/2026 Licensing Program Analysts (LPAs) Raquel Hernandez and Matthew Aguilar conducted an unannounced case management visit to follow up on complaint 56-AS-20260605120020. LPAs were granted entry to facility by Staff Joann Salem. LPAs met with Administrator Bene Molinitas and explained the purpose of today's visit.
During today's visit, LPAs received pertinent information in regard to complaint. LPAs conducted facility tour and did not observe any health or safety concerns. An exit interview was conducted and a copy of this report was provided to Administrator Bene Molinitas.
