Facility Evaluation Report
Licensing Program Analyst (LPA) Mary Rico made an unannounced visit to conduct a Health and Safety check of the clients in care at the facility. LPA Rico met with House Manager Glenn Bagro and explained the reason for the visit. LPA Rico attempted to contact Licensee Hershey Duenas, and a detailed voicemail was left. LPA Rico also contacted staff Natalie Eyler and explained the purpose of the interview.
The Health and Safety check included an overall observation of the facility inside and outside, including the food supply, medications, physical plant, and the residents in care. During the record review, LPA observed that R1 did not have their Admission Agreement, Pre-Admission Appraisal, or Care Plan. R2 did not have their Care Plan or Pre-Admission Appraisal. In addition, LPA observed that S1 and S2 have criminal record clearances that are not associated to the facility. A civil penalty of one thousand dollars was issued to the facility.
Based on the observations made during today’s visit, four deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and the report was discussed with and provided to House Manager Glenn Bargo. A copy of the appeal rights was also provided.
