Facility Evaluation Report
On 03/23/2026 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced case management visit based on an incident that occurred on 02/15/2026 pertaining to Client #1 (C1). LPA was granted entrance to the facility and explained the purpose of today's visit to Administrator Aichiro Funtila.
On 03/03/2026, the facility self-reported an incident that occurred on 02/15/2026 involving C1 getting hit by a car while riding their bike. C1 sustained no injuries and facility transported C1 to the hospital. LPA received hospital discharge paperwork.
LPA spoke with Administrator Aichiro Funtilla who indicated C1 is able to leave facility unassisted and when C1 leaves facility unassisted C1 has the administrator's contact information as well as facility's phone number. LPA reviewed C1's file as well as Physician Report (LIC602) that indicates C1 is able to leave facility unassisted. LPA attempted to interview C1, however, C1 was not at the facility during visit.
There were no deficiencies cited during today's visit and an exit interview was conducted with Administrator Aichiro Funtila to whom a copy of this report was provided.
