The inspector’s account

On 05/29/2026 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced Case Management incident visit. LPA met with Staff Lizeth Alonzo and explained the reason for today’s visit. The visit is pertaining to an incident that occurred on 04/26/2026 pertaining to Client #1 (C1).

On 04/28/2026, a special incident report (SIR) was submitted to licensing department in regards to possible neglect and dehydration happening to C1. During visit, LPA conducted (3) staff interviews. LPA observed C1 is non-verbal. LPA received additional information in regards to incident. LPA observed no imminent health or safety concerns. Facility had enough food and water available for all clients in care.

During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations regarding this incident. An exit interview was conducted and this report was provided to Staff Lizeth Alonzo.
