On 7/10/2026, Licensing Program Analyst (LPA) Mary Rico conducted an unannounced case management visit. LPA met with staff Kamala Hallare and Licensee Amend Qasim explained the purpose of the visit. The Lic
On 5/18/2026, the Licensee was cited for 85087(a)(3)(A) for having a family member living in the back shed. During today’s visit on 7/10/2026, LPA Rico toured the facility and observed that the family member had moved-out.
On 5/18/2026, the Licensee was cited on 80072(a)(2) for having locks on food pantry and a fence blocking access to the kitchen area. During today’s visit 7/10/2026, LPA Rico toured the facility and observed the fence was removed and the locks on the pantry. In addition, the following deficiencies were also corrected on today’s visit.
