Facility Evaluation Report
Licensing Program Analysts (LPAs) Ryan Gardner and Mary Rico arrived at the facility to conduct an unannounced case management visit. LPAs met with Caregiver Alyssa Perez and explained the reason for the visit. At the time of the visit, there were six (6) clients, and two (2) staff present.
During a prior visit on 3/10/23, LPA Gardner discovered there was a detached building on the facility property that was being rented out and occupied by two (2) occupants who are not affiliated with the facility. During this visit, the facility was given a deficiency and a plan of correction to vacate the two (2) occupants.
On a prior visit on 4/21/23, LPAs discovered that the two (2) occupants were residing and renting out the detached building on the facility property.
During today’s visit, LPAs discovered the facility has not vacated the two (2) occupants from the detached building. By continuing to allow the two (2) occupants to live on the property it poses an immediate health, safety, or personal rights risk to persons in care.
Based on observations today, a civil penalty in the amount of $1000.00 dollars will be issued for repeating the same violation within a twelve (12) month period. The facility will be issued one (1) type A deficiency per Title 22, Division 6, of the California Code of Regulations.
An exit interview was conducted, and this report (LIC809) was discussed and provided to Caregiver Alyssa Perez, along with a copy of LIC809D, LIC421MI, and the appeal rights.
