The inspector’s account

Licensing Program Analysts (LPAs) Ryan Gardner and Mary Rico arrived at the facility to conduct an unannounced case management visit. LPAs met with Caregiver Analyn Munguia and explained the reason for the visit. At the time of the visit, there were six (6) clients, and one (1) staff present.

During a prior visit on 3/10/23, LPA Gardner discovered there was a detached building on the facility property that was being rented out and occupied by two (2) occupants who are not affiliated with the facility. On 3/10/23, the facility was given a deficiency and a plan of correction to vacate the two (2) occupants.

During today’s visit, LPAs discovered the facility has not vacated the two (2) occupants from the detached building. By continuing to allow the two (2) occupants to live on the property it poses an immediate health, safety, or personal rights risk to persons in care.

Based on observations today, one (1) type A deficiency was cited per Title 22, Division 6, of the California Code of Regulations.

An exit interview was conducted, and this report (LIC809) was discussed and provided to Caregiver Analyn Munguia, along with a copy of LIC809D and the appeal rights.
