The inspector’s account

Licensing Program Analyst (LPA) E. Conchas made an unannounced case management visit to the facility for the purpose of conducting a health and safety check. LPA met with Administrator Lorena Martinez and explained the purpose of the visit. LPA requested pertinent documents and was provided with copies.

LPA conducted a tour of the interior and exterior of the facility. The facility was observed to have operable utilities, gas, electricity, and water measuring at above 120 degrees Fahrenheit. A sign was posted "Caution Hot".The washer was running when testing the temperature. LPA requested an updated water temperature from the residents restroom once the washer is not in use. Facility was observed to still provide care for the residents. The facility was observed to be clean. Residents were sitting in the living quarters watching a television show. The facility continues to have an adequate staffing level, LPA observed two (2) total staff, including the administrator. One (1) staff member was in the kitchen preparing food and another staff was attending to the residents and cleaning. LPA observed for the facility food supply to be sufficient for the number of residents in care, the facility has a 2 day supply of perishable and a 7 day supply of nonperishable food items. Residential files and medications, sharps and chemicals were observed to be in a secure area. One (1) fire extinguisher was observed current.

Administrator, Lorena Martinez stated is initiating a facility closure. The anticipated facility closure date is January 31, 2026. The residents’ responsible parties were provided with a sixty (60) day notice via email and mail on 11/26/2025. There are four (4) residents remaining in care. Per Lorena, three (3) of the four (4) residents responsible parties have confirmed the residents will be moving out by December the 31, 2025. Administrator confirmed that they will update LPA on resident’s whereabouts when they leave and will send Mike’s Angels LLC facility license to the Regional Office by closure date.

There were no health and safety concerns observed during today's visit.

An exit interview was conducted, and a copy of this report was provided to Administrator, Lorena Martinez.
