Facility Evaluation Report
On 10/2/2025 at 10:18 AM, Licensing Program Analyst (LPA) LaVette Farlow conducted an unannounced visit to the facility for the purpose of a Case Management visit health and safety check. LPA Farlow identified herself to Staff, Hortencia Alaniz and Arcelia Chavez De Aguile and discussed the purpose of the visit. No imminent health and/or safety concerns observed at the time of visit. LPA reviewed Personnel file, interviewed staff and residents and conducted a tour of the facility.
No deficiencies were observed during this visit. An exit interview was conducted where this report LIC809 was discussed and provided to Administrator, Lorena Martinez.
