Facility Evaluation Report

On 12/19/2025 at approximately 9:55 am Licensing Program Analyst (LPA) Sarina Ramirez arrived unannounced to the facility for an Annual inspection. Upon arrival, LPA observed clients (C1, C2, C3,C4) inside the facility with no staff present. Clients reported staff went down the street and would be expected to return shortly. LPA remained on site for about five minutes, during which time no staff were present to provide care and supervision.

At 10:00am LPA contacted Administrator by phone, who stated the staff must be down the street and would given them a call, Staff returned at about 10:10am.

Based on observations, client statements, and staff/administrator communication, the facility failed to ensure continuous care and supervision, posing an immediate health and safety risk to clients in care.

An exit interview was conducted with House Manager Raven Johnson. The deficiencies, civil penalty, and Plan of Correction were discussed. Appeal rights were provided. Copies of LIC 809, LIC 809D, and LIC 421 were issued.
