Facility Evaluation Report
Licensing Program Analyst (LPA) E. Conchas conducted an unannounced annual inspection required for the facility and met with Administrator Ivy Bader and discussed the purpose of the visit.
The facility is an Adult Residential Facility (ARF), level 2. The facility is an Inland Regional Center (IRC) certified vendor with a license capacity of (4) and a current census of (3). LPA conducted an overall inspection of the facility. The indoor and outdoor passageways are free of obstruction. Facility's backyard is enclosed and gated. Indoor and outdoor activity areas are sufficient for clients in care. Client activities include community outings and day program participation and personal entertainment. The facility has sufficient lighting and is maintained at a comfortable temperature of 71 degrees F. Client bathrooms were operating in sanitary conditions. The hot water temperature measured 112.8.- 116.6 degrees F. Client bedrooms have sufficient lighting and furniture in good repair. Facility has operating smoke detectors and carbon monoxide alarms, laundry equipment and telephone service. The facility has sufficient linens and personal hygiene items for clients in care. Facility was observed to have ammunition to be locked and inaccessible to clients. The facility has been posted in a common area: Administrator certificate, facility sketch, CCLD complaint poster, and house rules. The facility has sufficient non-perishable and perishable food supplies for clients in care. LPA observed emergency food supply and water. Sharps and chemicals were kept locked inaccessible to clients in care. Facility has 24-hour care staff. Staff working have criminal record clearances. Review of client files was observed to be complete. Review of staff files was observed to be incomplete although the required documents were observed to be current electronically. Infection control and emergency disaster plans were observed. Client’s medications are labeled and centrally stored in a locked cabinet.
Based on observations and record review, A technical violations were cited per Title 22, Division 6 of The California Code of Regulations.
An exit interview was conducted where the Licensing reports were discussed, and copies of the report were provided to Administrator Ivy Bader.
