Facility Evaluation Report
Licensing Program Analyst (LPA) Sarina Ramirez arrived at the facility unannounced to conduct a Case Management Visit for a health and safety. This case management visit is in response to a Special Incident Report (SIR) submitted to the Community Care Licensing Office on 03/12/2026. LPA met with Program Director Kathy Croke and stated purpose of the visit.
During today's visit, LPA discussed the incident and interviewed client #1 (C1). LPA will conduct further investigation in the matter.
No deficiencies were observed during this visit. An exit interview was conducted where this report was, reviewed, discussed and then provided to Program Director Kathy Croke at the conclusion visit.
