Facility Evaluation Report
On March 30, 2026, Licensing Program Analyst (LPA), Jarred Torres, arrived unannounced at the facility to conduct an interview with a client who attends their day program. LPA met with Administrator, Kathy Croke, and explained the purpose of the visit.
The purpose of the visit is related to complaint number 18-AS-20240618011025, which is not a complaint against the day program.
LPA conducted an interview with an alleged victim whom resides at an Adult Residential Facility to obtain information regarding the complaint number shown above.
