Facility Evaluation Report

Licensing Program Analyst (LPA) Amy Goldenberg is conducting this announced visit for the purpose of a pre-licensing inspection. LPA met with Kathy Croke, Director and Lisa Torres, LVN. This is an adult day program.

A fire clearance for 60 ambulatory clients was approved on 11/02/2021. LPA conducted a tour of the location. This is a change of ownership of an operating day program. LPA toured the facility and observed elements of the fire clearance, infection control, PPE supply, medication storage, sharp storage, cleaning supply and resident records. LPA measured the water in a client restroom at 105 degrees F.

Based on LPA observations there are no areas of concern observed per Title 22, Division 6, of the California Code of Regulations.
