On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unnanounced visit to the facility for the purpose of delivering an amended complaint investigation report for the complaint referenced with control number 22-AS-20220513105549.
LPA was greeted and granted entry by Administrator Mike Silverman after explaining the purpose of the visit. Amended report was signed and delivered, along with a copy of this report. Exit interview was provided.
