On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch conducted an unannounced case management visit to deliver an amended report for the complaint under control number 22-AS-20220513105549.
Amendment was performed to redact a confidential name, all other indications and investigation findings were left as delivered originally.
A copy of this report and the amended LIC9099-C were reviewed and left with facility representative.
