Facility Evaluation Report

At approximately 8:50AM, Licensing Program Analyst (LPA) Felias arrived unannounced to conduct a Case Management Incident Visit and met with Staff Member, Tandrea Nix. The purpose of the visit was to follow up on an SOC-341 report that was submitted to the Santa Rosa Regional Office (SRRO). During visit, Executive Director, Kate O'Connor and Program Operations Manager, Kayla Hotchkiss, arrived at approximately 9:25AM and 10:20AM. Executive Director left during visit at approximately 10:40AM.

An SOC-341 report was submitted to the SRRO on 07/24/2026. Report states that on 07/23/2026, facility management became aware of an incident of alleged abuse that occurred between Staff Member 1 (S1) and Client 1 (C1). Per report, the incident was observed by Staff Member 2 (S2) on 07/17/2026. Facility made all appropriate notifications per regulation.

LPA requested and obtained documents related to the incident.

Exit interview conducted. Copy of report discussed and provided to Program Operations Manager. Signature on form confirms receipt of documents.
