Facility Evaluation Report
On 06/02/2026 at approximately 12:30PM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Adult Residential Facility (ARF). Upon arrival, there were no one at the facility. LPA contacted the Administrator, Nakia Bibbs, and waited approximately 30 minutes for her arrival. LPA was informed that there were four (4) clients in care. The facility’s Fire Clearance has been approved for capacity of six (6) ambulatory clients. Facility is a single-story building with five (5) client bedrooms, two (2) bathrooms, and common spaces. During inspection all clients were attending a day program
At approximately 01:30PM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation.
At approximately 02:00 PM, the LPA and Administrator conducted a tour of the building and grounds, which were observed to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Toxins are stored in a locked cabinet in the garage. LPA observed sharp items used in food production kept with toxins and chemicals. LPA advised to keep kitchen items separate from toxins to avoid cross contamination (Technical Violation Given). Water temperature measured 111.6 degrees F and 109.8 degrees F which is within regulation between 105- and 120-degrees F at faucets accessible to clients. Fire Extinguishers found to be last charged in April 2026 at the time of visit. Carbon Monoxide and smoke detectors were present. There was enough lighting in all common areas, client rooms, and hallways. Medication is centrally stored and secure. However, there were some medications were not logged or documented on the Centrally Stored Medication log (See LIC809-D page).
