Facility Evaluation Report

Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Case Management Deficiencies in conjunction with a complaint visit (Complaint Control # 28-AS-20260406110743). The purpose of this visit is to issue deficiency that was observed by LPA that is not part of the complaint allegation.

During the visit on 04/07/2026, LPA discovered that the incident that occurred on Fri., 04/03/2026 involving Client #1 (C1) and staff members in which C1 had an aggressive and threatening behavior which caused C1 to be sent to the hospital but was not reported to CCLD.

Deficiency is noted on LIC 809D. Exit interview, a copy of this report and Appeals Rights were provided to Reyna Solis, Lead RBT.
